Utilize this form when requesting reimbursement for an expense paid, for a check request or for a deposit request for Administrative or Foundation fund expenses. Please follow the instructions on the form carefully to allow the treasurers to reimburse you in a timely manner.
We host many family/ kid-friendly events, and they are often photographed. This form may be filled out by parents/guardians, in order to allow TCMA permission to post photos of minors on its website and social media page(s).

Use this letter to verify the 501(c)3 status of the Travis County Medical Alliance Foundation. All donations to the TCMAF are tax deductible and purchases for the benefit of the TMCAF are tax free. Taxes paid are not reimbursable by the TCMAF.
